The Transition Survey is intended to collect parent input when planning for a special education student to transition after graduation. This document along with section 4 of the IEP can be used to help determine post-high school goals and what services/supports will be needed.
To fill this document out, either meet with the parents to discuss, or share the document with the parents and have them respond.
