Discipline Removal Services

This form helps districts meet the requirements of SERVICES DURING PERIODS OF REMOVAL. It is divided into the following sections:

• Services - See SERVICES DURING PERIODS OF REMOVAL
• Signatures - Documents participants. If removal is a change of placement, an ARD committee, rather than school personnel only, is required per SERVICES DURING PERIODS OF REMOVAL
