This form helps districts meet the requirements of ADULT STUDENT AND TRANSFER OF RIGHTS: NOTIFICATION OF THE TRANSFER OF RIGHTS BY AGE 17. It is the Notice of Transfer of Rights - Model Form published by TEA, with information and resources regarding:

• Guardianship
• Alternatives to Guardianship
• Supported-Decision Making Agreement

This form informs parents and students that rights WILL TRANSFER at age 18.
