Ohio Section 504 Form Updates

We recently released the following updates to all districts:

• Section 504 EMIS - Added an additional view to the “Section 504: Plan” entitled "EMIS at a Glance (504 Plan)"
• Section 504 Rights - Added an additional view to many of the Section 504 forms (Referral, Prior Written Notice, Consent, Evaluation, Plan) entitled "Notice of Parent/Student Rights" to more easily communicate procedural safeguards for 504.
  • This additional view has 2 boxes - one for the district's 504 coordinator, and one for the OCR regional office. To populate both boxes (for all forms), visit Settings (left menu) > District Information (Details tab). Scroll to the bottom of the page until you see "SECTION 504 PROCEDURAL SAFEGUARDS CONTACT INFORMATION". Add information to the two corresponding textboxes.
