Services Plans

When a student is attending private school but requires special education services, you may create a Services Plan for them.

When a student has a Services Plan, they have access to some special education services, but not all of the same services as public school students.

The Services Plan form is very similar to an Individualized Education Program but without the following sections:

• State and Districtwide Assessments
• Consideration of Least Restrictive Environment
• End of School Year (ESY) Services
• Assurances

To fill out a Services Plan, follow the workflow to fill out an IEP, but skip the sections listed above.

To amend or revise a Services plan, use the same amendment or revision process as an IEP.
