Parental Consent for Evaluation for HB 548 Language Acquisition

TEA provides state guidance for AI and VI students. This form is based on guidance published by TEA for collecting parent consent to assess the language acquisition of students aged 0-8 years old who are deaf or hard of hearing. A similar form is provided with Section 504 forms.
