Physician's Verification of Other Health Impairment

This form helps districts meet the requirements of OTHER HEALTH IMPAIRMENT. In order for a child to be determined to have an other health impairment, the disability must be verified by one of the following:

• Licensed physician
• Physician assistant
• Advanced practice registered nurse with authority delegated under Texas Occupations Code, Chapter 157
