In SameGoal, you can use an LPAC Decisions Regarding Assessments document to help assess a student's understanding of different topics to determine what assessments and accommodations they need.

1. Create an LPAC Decisions Regarding Assessments.
   • There are two form variants; one for Grades K-5 and one for Grades 6-12. Use the correct variant based on student grade.

     If you're creating multiple of these documents at once, you may choose to bulk create them.
2. Enter assessment and accommodation information.
3. Share the document with required educators for signatures.
4. Mark document complete.

If your district uses a TestHound integration, assessment requirements and accommodations from these forms will flow to TestHound once the document is marked complete (integration runs nightly).
